For all packages, an additional .99 cents will be billed for each minute used over each package allotment with the exception of ** Executive Office Service, where additional minutes are billed at .75 per minute. All patched calls will not be counted against the minutes included in each plan, but will incur a separate charge of .05 cents per minute. The hours of service are 8:30am – Midnight Monday through Friday, and 10:00am - 3:00pm Saturday (Eastern Standard Time). During hours in which the office is closed, all calls will be answered by a private voicemail box which is accessible by Client 24 hours a day, 7 days a week and is included with the service.

As part of SO’s corporate identity program Client may use 1 or 2 (depending on package) of the following addresses as their own. Client should return form (PS Form 1583) with all sections that apply completed, leaving #’s 3, 4, and 5 blank. In addition, Client must provide SO with two copies (front and back) of a photo I.D., as well as two copies of one other form of I.D. (see PS Form 1583 for details) and have a notary public sign section 15. Without receipt of these forms, SO cannot receive and then forward any mail to Client.
